Surrender of Chinchilla Form
We do not require a fee to take in your chinchilla, but we are going to ask that ALL supplies that you currently have for your chinchilla (cage, toys, food, bedding, etc) be surrendered along with your chinchilla- WE WILL NOT pay you for these items.
Your chinchilla deserves to be kept as comfortable as possible during this stressful time and providing them with a familiar cage and toys will help during this process.
We will also request that you make the effort to drop your chinchilla off at an arranged time at our home. We may be able to arrange to meet you at a more neutral location, but we would prefer for you to make the effort and bring your chinchilla to us.

By signing below you are acknowledging that you are surrendering your chinchilla to us by your own request to do so. You are also acknowledging that you agree to the re-homing of your chinchilla to a new home and that you will not get your chinchilla back. Should you decide that you want your chinchilla back after surrendering it you will be required to pay the re-homing fee and you must also fill out an adoption form and prove you qualify as an acceptable home for a chinchilla.
